Following forms are REQUIRED to fulfill registration requirements:
Participants

PROOF of age - Copy of Certified birth certificate

2008 Steeler Registration
3. 2008 Pop Warner Participant
Contract
4. 2008
Medical/Physical Form (**must be dated after January 1, 2008!)
5. 2007-2008 COMPLETE report card. (accepted after the school year ends)

Participants will be allowed on the practice field without ALL
required paperwork.

Volunteer Application (with Cover) **MUST be accompanied by a photocopy
of your drivers license (3 pages needed to process)
